I think some banks (HSBC for example) issue T5008 for their customers who trade stocks, but I am not sure if every bank does the same.

And I think that it's no different for you to input your txn in Sch 3 or T5008 form, coz the data in t5008 will be shown on Sch3.  The most important thing is that CRA knows how to reach you if it needs more details.

When you net-file, CRA does not receive the T5008 but the Schedule 3. It might be ok to add the missing ACB on the T5008. But, I usually disregard the T5008 and enter everything on the Schedule 3. The securities you sold are identified on the T5008 and CRA can verify if you reported them on the S3.

not sure how trustworthy they are, but yes I did not file T5008 through Netfile and I had no problem.
